1. Bring your ID preferably your driver's license to your physical exam.  You will need the number. 

2. Bring a utility bill with your new address if different from the one on your driver's license..

3. Have your medical history; including any medicines you might be taking.

4. Take you medicines on schedule, if applicable.

Wear corrective lenses for long distance, if applicable.

NB. Admitting to drug use (including marijuana) DISQUALIFIES you.
